Group Homes

Group Homes Form

Description: Complete this questionnaire as the first step to opening and operating a group home in an existing building. The questionnaire will help staff determine whether you will need a building permit for a change of use or whether a certificate of occupancy can be issued without a permit. Once submitted, staff will review and contact you directly with next steps.
